29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).      On or about January 30, 2013, on the west side of the construction site located at 4841 Merlot Avenue, Grapevine, Texas, an employee was exposed to a fall hazard of approximately 10 feet while engaged in decking activities on a residential home.

29 CFR 1926.503(a)(2)(i): Employees were not trained, as necessary, by a competent person qualified to recognize the nature of fall hazards in the work area.       On or about January 30, 2013, on the west side of the construction site, located at 4841 Merlot Avenue, Grapevine, Texas, an employee was exposed to a fall hazard of approximately 10 feet while engaged in decking activities on a residential home.  The employee had not been adequately trained to recognize and avoid fall related hazards.
