29 CFR 1910.133(a)(1):     The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    On or about February 21, 2013 and times prior thereto, employees of Southworth Products Corp. performed welding and grinding operations for the manufacture of ergonomic material handling equipment in the Light Customs Welding Area and the Matrix Welding Area of the employer's facility located at 715 S. Highway 77 in Manila, Arkansas.  The employees performing grinding and welding operations and employees walking or working near these operations were exposed to flying metal debris to the eyes due to not being provided the appropriate eye protection for this hazard.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about February 21, 2013 and times prior thereto, employees of Southworth Products Corp. operated a yellow hydraulic press brake located in the Clean Room of the employer's facility located at 715 S. Hwy 77 in Manila, Arkansas.  The hydraulic press brake observed was used by employees to press bushings into rods on cylinders and was foot pedal operated with no guarding at the point of operation, exposing employees to amputation hazards.

29 CFR 1910.138(b):     The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified:    On or about February 21, 2013 and times prior thereto, employees of Southworth Products Corp. performed metal inert gas (MIG) welding for the manufacture of ergonomic material handling equipment in the Light Customs Welding Area and the Matrix Welding Area of the employer's facility located at 715 S. Highway 77 in Manila, Arkansas.  The employer had provided employees with Radnor large select shoulder grade split leather palm gloves (Part Number: RAD64057525) which were not the appropriate hand protection for welding operations.
