29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    The employer does not ensure that open-sided floor(s) or platform(s) 4 feet or more above the ground are guarded by railing on all open sides.  This violation was observed on or about February 7, 2013, on an unguarded overhead storage space above the Shop floor where an employee was exposed to a fall hazard of more than 4 feet when retrieving a fence for the table saw.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level are guarded by standard railings on all open sides.

This violation was observed on or about February 7, 2013, on the Northeast side of the Shop where employees were exposed to a struck-by hazard when operating a table saw without a guard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guards are installed on all table saws to protect them from the point of operation.

This violation was observed on or about February 7, 2013, on the Northeast side of the Shop where employees were exposed to a struck-by hazard when operating a table saw without a spreader.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that spreaders are installed on all table saws to protect them from hazards originating from the point of operation.

This violation was observed on or about February 7, 2013, on the Northeast side of the Shop where employees were exposed to a struck-by hazard when operating a table saw without having a kickback device installed on it.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that kickback devices are installed on all table saws to protect them from hazards originating from the point of operation.

29 CFR 1910.305(b)(2)(i):     Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  The employer does not provide a cover for each outlet box in completed electrical installations:      a.	This violation was observed on or about February 7, 2013 at the North wall of the Main Shop where employees were exposed to an electrical shock/burn hazard when operating the DeWalt Chop Saw whereas the electrical outlet box had no face plate.    b.	This violation was observed on or about February 7, on the North wall of the Main Shop near the ABC fire extinguisher where employees were exposed to an electrical shock/burn hazard when operating a DeWalt Chop Saw near an electrical outlet box which had no face plate on it.     c.	This violation was observed on or about February 7, on the North wall of the Main Shop where employees were exposed to an electrical shock /burn hazard when operating the DeWalt Chop Saw near a junction box which did not have a metal cover on it.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all outlet boxes and junction boxes in completed installations have a face plate or cover on them.
