29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) 1800 Warnock Street, Electrical service panel - On or about 2/23/2013, employees, who had been performing caulking and waterproofing, were not protected against electric shock when they removed the locked deadfront cover from the electric service panel for access to switch off the construction lights. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: (a) 1800 Warnock Street, Electric service panel - On or about 2/23/2013, employees, who had been performing caulking and waterproofing, were exposed to energized electrical conductors without protective shields, barriers or personal protective equipment when the employees removed an electric panel dead front to access the circuit breakers to turn off the construction lighting. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
