29 CFR 1910.134(c)(1):  A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:        a)  On 2/27/2013, the employer had not developed a written respirator protection program for torcher employees who were required to wear tight-fitting half-face respirators while oxygen/propane torch cutting ferrous scrap metal at the Alter Metal Recycling facility located in Waupaca, WI.        All provisions of 29 CFR 1910.134(c) through (m) must be covered in a written respiratory protection program. Key elements include, but are not limited to the following:        1)  Medical evaluation   2)  Fit testing   3)  Respirator selection  4)  Change schedule for filter cartridges   5)  Maintenance, use and care of respirator   6)  Employee training   7)  Recordkeeping        ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for respirators not equipped with an End of Service Life Indicator that would ensure cartridges were changed before the end of their service life:  a)  On 2/27/2013, the employer did not implement a change schedule for employee respirators, which were not equipped with an End of Service Life Indicator when torcher employees were required to wear tight-fitting half-face respirators, while oxygen/propane torch cutting ferrous scrap metal at the Alter Metal Recycling facility located in Waupaca, WI.   Note:  The employer may provide an air-purifying respirator provided the respirator is equipped with end-of-service-life indicators or a change schedule has been developed. Otherwise, the employer must provide an atmosphere-supplying respirator.  ABATEMENT CERTIFICATION REQUIRED.
