29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) 83 Warwick St Springfield, MA: On or about February 26, 2013, employees were observed installing photovotalic panels on this flat roof more than 25 Ft above the ground below without any fall protection as required by this subpart.

29 CFR 1926.1053(a)(24):The side rails of through or side-step fixed ladders did not extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder:    (a) 83 Warwick St Springfield, MA: On or about February 26, 2013, employees had to climb the fixed ladder to access the roof work area which was 2.2 feet shy of the top of the roof access panel and did not extend any distance past the landing served by this ladder which is not in accordance with this subpartsubpart.
