At the workplace, on or about March 12, 2013, an employee moving a GPU Coil with a Toyota forklift had an unstable load on the forklift, resulting in an amputation.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. At the workplace, in the cutting area, the Motoman cutting robot was not guarded to prevent employees from a struck by hazard of the moving and rotating parts. b. At the workplace, in the cutting area, the DoALL Metal Cutting Bandsaw, Model C-9, Serial Number 260-68136, had an unused portion of the blade that was not guarded exposing employees to an amputation hazard.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a. At the workplace, in the welding area, a pipe roller had chains and sprockets near the electric motor, approximately three and a half feet above the floor, that were not guarded exposing employees to an amputation hazard.
