Production Area - On or about March 1, 2013, employees were potentially exposed to flying fragments in the event of an abrasive wheel wheel failure.  The abrasive wheel on the DeWalt DW758 bench grinder was not used and maintained in a safe manner.  Employees were using the grinder improperly by, among other methods, side grinding, causing furrows to develop on the abrasive wheel.  The wheel was in need of proper dressing.
