The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught in-between and crushed by hazards: On or about January 26, 2013, at the inclined loading dock of the old hospital, at 605 N 12th ST, Mt. Vernon IL. Employees were exposed to the hazard of being caught in-between and crushed while operating lift gates behind the box trucks that were not secured by some positive means such as , but not limited to, parking brakes and/or wheel chocks. Among other methods, feasible abatement methods include, among others, would be to: * Develop, implement, and ensure compliance with a work procedure which adheres to the vehicle`s liftgate manufacturer`s instructions which states, "Be certain vehicle is properly and securely braked before using the liftgate." * Develop, implement, and ensure compliance with a work procedure which adheres to the Illinois Rules of the Road for Non-CDL Vehicles which requires a regular inspection of all vehicle equipment including by not limited to brake, liftgate, and backup alarm function. * Ensure that provide vehicle safety equipment such as "back-up alarms" and parking brakes are functional and tested on a frequent and regular basis.
