29 CFR 1926.501(b)(13):    Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    a) Construction Site: On or about March 05, 2013, and at times prior thereto, employees were exposed to a fall of approximately twenty feet to the ground below while sheathing the roof of a new residential home.

29 CFR 1926.502(b)(2):     Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:      a) Construction Site: On or about March 05, 2013, and at times prior thereto, employees were exposed to a fall of approximately nine feet to the floor below while walking in and around the floor hole at the top of the stairway. The guard rail system was missing a mid-rail.

29 CFR 1926.503(a)(1):     The employer did not provide an effective training program for each employee who was exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:      a) Construction Site: On or about March 05, 2013, and at times prior thereto, employees were exposed to a fall of approximately twenty feet to the ground below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
