Production Abrasives, Inc. - On or about 3/5/13, employees were exposed to crush hazards from being ejected from powered industrial trucks (Hyster; Serial #C114V06195P & CAT; MC30) due to them not being equipped with seat belts.  The fork trucks were being utilized to move rolls of abrasive material.

29 CFR 1910.151(c):     Where the eyes or body of any person were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:          A.)  Production Abrasives, Inc. - On or about 3/5/13, employees were charging an electric forklift (CAT; Model #MC30) in an area that did not have adequate flushing facilities.  The area had two (2) eye wash water bottles which did not meet the 15 minute continuous flow requirements for eye washes.

Production Abrasives, Inc. - On or about 3/5/13, employees were utilizing a man basket on forklift (CAT Model #MC30) without manufacturer's approval.  The man basket was used for activities including but not limited to, to lift employees to change light bulbs in the facility and retrieve overhead storage items.
