29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  The employer does not fully plank or deck between the front uprights and guardrail supports.  This violation was observed on or about March 4, 2013 on the southeast corner of the construction site where employees were exposed to an approximate 10 foot 3 inch fall hazard when performing masonry work without the working level being fully planked.  Pursuant to 29 C.F.R. 1903.19 within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls  6 feet or more above a lower level.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  The employer does not ensure that scaffold poles rested on a base plate.  This violation was observed on or about March 4, 2013 on the southeast corner of the construction site where employees were exposed to an approximate 10 foot 3 inch fall hazard when performing masonry work when one scaffold support pole did not rest on a base plate.   Pursuant to 29 C.F.R. 1903.19 within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from fall hazards when one scaffold leg did not rest upon a base plate.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:   The employer does not ensure that a portable ladder, hook-on ladder, attachable ladder, stair tower (scaffold stairways/towers), stairway-type ladder (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was used with a scaffold platform more than 2 feet above a point of access.  This violation was observed on or about March 4, 2013 on the southeast corner of the construction site where employees were exposed to an approximate 10 foot  3 inch fall hazard when performing masonry work without using a safe means of access to and from the scaffold platform which was more than 2 feet above or below a point of access.   Pursuant to 29 C.F.R. 1903.19 within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls 2 feet above or below a point of access on a scaffold.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  The employer does not ensure that employees on scaffold more than 10 feet above a lower level are protected from falling to that lower level by fall protection.  This violation was observed on or about March 4, 2013 on the southeast corner of the construction site where employees were exposed to an approximate 10 foot 3 inch fall hazard when performing masonry work without fall protection.   Pursuant to 29 C.F.R. 1903.19 within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from fall hazards above 10 feet.
