29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, in-going nip points, rotating parts, flying chips and sparks:      (a)   Alumalloy Metalcasting Co., Avon Lake, Ohio:  On or about March, 5, 2013, the employer did not utilize one or more methods to guard the unused portion of the band saw blade.  The adjustable guard was raised an estimated 8 inches above the largest dimension of the casting exposing the employee to a cut and amputation hazard.  The employees hands repeatedly were within 4 inches of the moving blade.
