29 CFR 1910.22(a)(2):     The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.     On or about March 6, 2013 and at times prior thereto, at the Back Track Trailer Division, the employer did not ensure the workroom floor was maintained in a dry condition. Employees performing work-related activities such as but not limited to tig and mig welding and operating a punch press and bandsaw were allowed to work in approximately 2-3 inches of water arising from roof leaks and exit and overhead door gaps at floor level. This created a condition which exposed employees to slip and fall, electrical shock and/or burn and electrocution hazards.

29 CFR 1910.37(a)(3):     Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.     On or about March 6, 2013, at the Back Track Trailer Division, the employer did not ensure the exit route was free and unobstructed. A mobile rack and metal shopping carts with boat parts and other supplies were located within the exit route blocking the path to the exit door. This condition created delay in exiting the building in an emergency, and  trip and fall hazards in a fire.

This condition creates delay in exiting the building in an emergency, and  trip and fall hazards in a fire.     Instance b:  At the east exterior wall, the exit door is missing  its door knob and was barely hinged by a small sliding door latch. This condition creates delay in exiting the building in an emergency.

On or about March 6, 2013, at the Back Track Trailer Division, the employer allowed the use of a temporary flexible cord to be used as a substitute for the fixed wiring of the structure. Employees were using a surge protector extension cord to power a radio/cd player.   This condition exposed employees to electrical shock and/or burn and fire hazards.
