29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use: There was no written Respiratory Protection program at the facility for chemists working in the lab who were required by the employer to wear 1/2 mask negative-pressure respirators. Note that employees/chemists were not fit-tested.

29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons shall be adequately trained to render first aid. Adequate first aid supplies shall be readily available: There was no one trained in first aid on the second and third shifts. The local ambulance's response time was 6-8 minutes. Note: After the assignment of first aid providers on all three shifts an exposure control program will be required.

29 CFR 1910.132(f)(1): The employer shall provide training to each employee who is required by this section to use PPE: Training was not provided to employees required to use personal protective equipment that was identified in the workplace hazard assessment performed on or about 07/27/2011, including but not limited to protective sleeves, cut-resistant gloves, goggles, high-temperature gloves, face-shields, and coveralls. Training shall include the following: 1. When PPE is necessary; 2. What PPE is necessary; 3. How to properly don, doff, adjust, and wear PPE; 4. The limitations of the PPE; and, 5. The proper care, maintenance, useful life and disposal of the PPE. Training shall be verified by a written certification that includes the name of each employee trained, the dates of the training, and the subject of the certification. PREMIERE INDUSTRIAL SUPPLY INC.
