29 CFR 1910.1030(c)(1)(iv):     The  Exposure Control Plan was not reviewed and updated by the facility at least annually and whenever necessary to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure.     Facility-The plan was developed at the corporate level and called for annual review by the "administrator or designee".  To "assist the facility in complying...." (Section 3.0),  the review process provided for signed and dated certifications (page 3); bloodborne hazard assessment and exposure w/associated forms (pg 29 and Appendix C).  None of these had been accomplished as required by the standard and/or the facility plan.
