29 CFR 1910.178(m)(10): A load backrest extension shall be used whenever necessary to minimize the possibility of the load or part of it from falling rearward. Warehouse - On or about March 7, 2013, the Komatsu (serial #181171A) sit down forklift truck was not equipped with a load backrest extension. Employees were thereby exposed to materials falling backward.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. Warehouse - On or about March 7, 2013, the horn was missing from on the Komatsu (serial #181181A) powered industrial forkilift truck and was not removed from service.

29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. Warehouse - On or about March 7, 2013, skids of materials stored on the floor were not secured to prevent from sliding and collapsing, thereby exposing employees to crushed by hazards.
