29 CFR 1910.305(e)(2):  Switches, circuit breakers and switchboards installed in wet locations were not enclosed in weatherproof enclosures:   In the residents' shower room located between hallway 500 and hallway 600, the double light switch on the tiled partition wall between the two shower stalls, the double light switch and faceplate are not sealed and waterproof.  Employees showering residents are exposed to electrical shock when turning the light switches on and off.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:   At Retama Manor Nursing Center - Robstown, the employer did not correctly record an injury on the OSHA Form 300  or equivalent for the calendar year 2010.  An employee suffered a knee injury in calendar year 2010 and underwent knee surgery in calendar year 2011.  The employer did not update the OSHA Form 300 for calendar year 2010 by entering a check mark in column H or indicating the number of days in column K, when the case resulted in days away from work.

29 CFR 1910.303(f)(2):  Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  In the electrical closet adjacent to the laundry room, electrical panels #1, #2, and #3 did not have the circuit breakers marked as to their function or purpose.
