29 CFR 1910.134(e)(1):  The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:     a)  At the facility, on or about March 7, 2013, and at times prior to, where the employer did not ensure that employees were provided with a medical evaluation prior to using 3M half-face respirators.  Employees used the half-face respirators to perform spray painting tasks at the facility.  This condition exposed employees to respiratory hazards.
