29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    On or about March 7, 2013, at the above addressed jobsite, an employee performing work picking up roofing materials directly under two other employees performing roof work was not wearing head protection equipment while exposed to potential struck by head injury.

29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about March 7, 2013, at the above addressed jobsite:  Employees performing roof work installing metal flashing on a steep roof with unprotected edges approximately 14 ft. above a lower level were not protected from falling to that lower level.

29 CFR 1926.1053(b)(8):  Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    On or about March 7, 2013, at the above addressed jobsite:  Employees gaining access to and from a roof approximately 20 ft. above a lower level using an 28 ft. fiberglass extension ladder set up on an active parking lot and walkway did not have that ladder secured to prevent accidental displacement.
