29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a)  Kasab Multi Service, Inc. operating at 925 Hilltop Drive, Itasca, IL:  On or about March 7, 2013, an employee was conducting repair operations to an exhaust fan while on top of a curing oven.  The employee was exposed to a fall hazard of about 11.5 feet without fall protection.    There is no abatement certification or documentation required for this item.
