OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees working near a conveyor system with an inoperable emergency stop device (detached emergency stop pull cord).  a) Cyclone Feed Conveyor carrying discharge materials from Svedala 6080 Hammermill Metal Shredder, Rear 9 Fellows Avenue, Wilkes-Barre PA: Employees engaged in dislodging metal causing blockages at the loading end of the operating Cyclone Feed Conveyor that had a detached and inoperable emergency stop pull cord. Employees were exposed to amputation hazards and could not immediately stop the conveyor in the event of an emergency, on or about March 8, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

Employees were exposed to a fall hazard of approximately seven-foot, two-inches (7'2"). b) South Side of Shredder Concrete Pad, Rear 9 Fellows Avenue, Hanover Township, PA: An open-sided platform more than four-foot above ground level was not guarded with standard railings (or equivalent). 29 CFR 1910.147(c)(4)(i) Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) Cyclone Feed Conveyor carrying discharge materials from Svedala 6080 Hammermill Metal Shredder, Rear 9 Fellows Avenue, Wilkes-Barre PA: The employer did not have a procedure developed, documented and utilized to isolate the electrical power supply when employees were engaged in dislodging metal causing blockages at the loading end of the Cyclone Feed Conveyor. Employees were exposed to amputation hazards in the event of an unexpected startup, on or about March 8, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

Employees were exposed to amputation hazards, on or about March 8, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
