29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcture, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:    a) South side of building, front, on or about 3-7-13:  Two employees were accessing multiple levels of a scaffold while climbing the x-braces, exposing the employees to a fall hazard of 20 feet 6 inches.

29 CFR 1926.451(g)(4)(xv):  Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart:    a) South side of building, front, on or about 3-7-13:  Two employees were working on a scaffold platform that used cross braces as a full guard rail system, exposing the employee to a fall hazard of 20 feet 6 inches.
