29 CFR 1910.1000(a)(2): Employees were exposure to Carbon Monoxide listed in Table Z-1, in excess of the 8-hour Time Weighted Average concentration of 50 ppm in 8-hour work shift of a 40-hour work week.   a.) On or about 03/08/13, an employee was exposed to 8Hr-TWA Carbon Monoxide level of  90.7 ppm.  b.) On or about 03/08/13, an employee was exposed to 8Hr-TWA Carbon Monoxide level of 114.7 ppm.  c.) On or about 03/08/13, an employee was exposed to 8Hr-TWA Carbon Monoxide level of 124.9 ppm.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   On or about 03/15/13, employees were not provided with training on hazardous chemicals such as but not limited to Carbon Monoxide, and products containing ammonium chlorides, ethylene diamine tetraacetic and others.
