29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Location: Parker Drive    The employer did not develop, document and enforce a lockout/tagout procedure for employees servicing vehicles.  An employee was fatality injured while working on the brakes on a running tractor trailer unit, where the driver was unaware of the mechanic under the vehicle.  The driver relocated the vehicle crushing the mechanic.
