29 CFR 1910.178(q)(6): Industrial trucks were altered by the addition of extra parts not provided by the manufacturer.  (a) Denver Plastics Colorado, 560 Dahlia Street, Hudson, CO 80642; On and before March 12, 2013, the exposing employer allowed an attachment to be placed on a powered industrial vehicle (forklift) which was not originally received from the manufacturer. Specifically, the employer designed and assembled a block and swivel device which slides over the forks and is used to lift cast iron plastic molds weighing up to 3000 lbs from their storage into position in the hydraulic injection mold machines.   Condition exposed employees to struck by and crush hazards as they used the forkligt to change out molds from the plastic injection machines.
