29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) At the site located at 2113 Crocker Street, Houston, TX: On March 4, 2013 employees conducting siding approximately 37 feet 10 inches above the ground were working on a pump jack scaffold missing guardrails on the side and ends, exposing employees to a fall hazard.

29 CFR 1926.452(j)(1): 29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories were not fabricated from metal plates and angles. Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage: a) At the site located at 2113 Crocker Street, Houston, TX: On March 4, 2013 were observed working 37 feet above ground on a pump jack scaffold with bracing made out of plywood and lumber nailed together, exposing employees to a fall hazard.
