29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:     a)  At the site located at 2113 Crocker Street, Houston, TX: On March 4, 2013 employees conducting siding approximately 37 feet 10 inches above the ground were working on a pump jack scaffold missing guardrails on the side and ends, exposing employees to a fall hazard.

Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage:   a)  At the site located at 2113 Crocker Street, Houston, TX: On March 4, 2013 were observed working 37 feet above ground on a pump jack scaffold with bracing made out of plywood and lumber nailed together, exposing employees to a fall hazard.
