(a)  The primary chemical mixer working in the mixing booth 4-6 hours a day did not have a medical evaluation conducted prior to his fit test (date of his medical evaluation:  2/21/13; date of his fit test:  9/21/12).    (b)  The back-up mixer working in the mixing booth approximately 2 hours a day did not have a medical evaluation conducted prior to his fit test (date of his medical evaluation:  1/3/13; date of his fit test: 12/21/12).
