Recovery Room - On October 4, 2012, an employee sustained a needlestick percutaneous injury after she had administered an I.M. injection of Zofran to a vomiting patient. While she was discarding a used Select Medical Product 22 G x 1 inch needle (Cat. No. 443) into a sharps container, it accidentally slipped and struck her middle left finger. The employer did not document in their written Exposure Control Plan annual consideration of appropriate commercially available and effective safer medical devices for injections. Observed On or About March 13, 2013

29 CFR 1910.1030(c)(1)(v): An employer, who is required to establish an Exposure Control Plan did not solicit input from non-managerial employees responsible for direct patient care who are potentially exposed to injuries from contaminated sharps in the identification, evaluation, and selection of effective engineering and work practice controls. a. Recovery Room - On October 4, 2012, an employee sustained a needlestick percutaneous injury after she had administered an I.M. injection of Zofran to a vomiting patient. While she was discarding a used Select Medical Product 22 G x 1 inch needle (Cat. No. 443) into a sharps container, it accidentally slipped and struck her middle left finger. The employer did not document the solicitation of input from non-managerial employees responsible for direct patient care who are potentially exposed to injuries from contaminated sharps in the identification, evaluation, and selection of effective engineering and work practice controls in the Exposure Control Plan. Staff involvement in the device selection and evaluation process is an important element in the prevention of percutaneous injuries from needles or other sharps contaminated with bloodborne pathogens, such as, human immunodeficiency virus (HIV), hepatitis B (HBV), and hepatitis C (HCV). 29 CFR 1910.1030(d)(2)(i): Engineering and work practice controls were not used to eliminate or minimize employee exposure. Where occupational exposure remains after institution of these controls, personal protective equipment shall also be used. a. Recovery Room - An employee stated that she used either a Select Medical Products 18G x 1 inch needle (Cat. No. 433) or a Select Medical Products 3cc Syringe with 22G x 1 inch needle to administer intramuscular (I.M.) injections of Depo-Provera Medroxyprogesterone Acetate (NDC Code, 59762-4537-1), progestogen-only contraceptive, to patients. These needles were not equipped with built-in safety features designed either to eliminate or to minimize the risk of accidental percutaneous injuries from needles contaminated with bloodborne pathogens, such as, human immunodeficiency virus (HIV), hepatitis B (HBV), and hepatitis C (HCV). Observed On or About March 13, 2013
