29 CFR 1910.1030(c)(1)(v): An employer, who is required to establish an Exposure Control Plan did not solicit input from non-managerial employees responsible for direct patient care who are potentially exposed to injuries from contaminated sharps in the identification, evaluation, and selection of effective engineering and work practice controls.   a. 29 CFR 1910.1030(d)(2)(i): Engineering and work practice controls were not used to eliminate or minimize employee exposure. Where occupational exposure remains after institution of these controls, personal protective equipment shall also be used.   a. Recovery Room - An employee stated that she used either a Select Medical Products 18G x 1 inch needle (Cat. No. 433) or a Select Medical Products 3cc Syringe with 22G x 1 inch needle to administer intramuscular (I.M.) injections of Depo-Provera Medroxyprogesterone Acetate (NDC Code, 59762-4537-1), progestogen-only contraceptive, to patients.  These needles were not equipped with built-in safety features designed either to eliminate or to minimize the risk of accidental percutaneous injuries from needles contaminated with bloodborne pathogens, such as, human immunodeficiency virus (HIV), hepatitis B (HBV), and hepatitis C (HCV).  Observed On or About March 13, 2013
