29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) Employees were exposed to an approximate 14 foot fall hazards while working off of sheets of plywood, which were being elevated by a Terex SS-842 Rough Terrain Forklift and the employer did not ensure that scaffold planks were attached to the forks.
