29 CFR 1910.219(f)(3):     Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed.    On or about 13 March 2013 employees of Weatherford Composites were exposed to amputation, caught in, and pinching hazards while working near an unguarded sprocket and chain on winding machine 6 located in the winding department.

29 CFR 1910.134 App D:     The employer shall provide the basic advisory information on respirators, as presented in Appendix D of this section, in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer.    On or about 13 March 2013 employees of Weaterford Composites were exposed to respiratory hazards while voluntarilly wearing respirators.
