29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about March 12, 2013, at the worksite located at 710 West Main in Cabot, Arkansas, east side of building being constructed, a safe means of egress was not provided for employees working in an approximately 3 ft. wide x 40 ft. long x 54" deep excavation.  This condition exposed employees to the hazard of being struck by falling excavated materials and/or cave-ins from not being able to egress from the excavation.
