Cement Batch Plant - On or about March 13, 2013, and at times prior thereto, employees were exposed to stairway that did not have standard rails (i.e. top rail and mid rail) on the open side of the stairs.

Cement Batch Plant - On or about March 13, 2013, and at times prior thereto, company employees were worked with and around hazards substances inculding, but not limited to, crystalline silica while loading cement trucks. The company had not written and implemented a Hazard Communication Program.

Cement Batch Plant - On or about March 13, 2013, and at times prior thereto, company employees were potentially exposed to crystalline silica while loading cement trucks.
