29 CFR 1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:     a) On or about March 22, 2013, at the above located workplace, employees required to work with a 120 volt wheel chair washer, were exposed to potential electrical hazards from a 120 volt power supply cord which was missing the grounding pin. The condition of the cord was not in accordance with instructions included in the listing or labeling. The employees were thereby exposed to the hazards associated with electrical shock.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) Between approximately February 1, 2013 and March 25, 2013, at the above located workplace, employees in the laundry area were required to use Ecolab Laundi Destainer a corrosive chemical. The laundry work area did not have an eyewash capable of providing 15 minutes of water to both eyes.    b) Between approximately February 1, 2013 and March 25, 2013, at the above located workplace, employees in the kitchen were required to use ChemPro Oven & Grill Cleaner a corrosive chemical. The kitchen work area did not have an eyewash capable of providing 15 minutes of water to both eyes.
