29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):      Employees working on an approximately 18 foot high, with a pitch of 5 in 12 roof without any fall protection system in use exposing employees to fall hazards.  The  employer did not initiate inspections by a competent person to control and eliminate the hazards present at the jobsite in accordance with OSHA regulations.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:         a)    Employees working on a 5 in 12 pitch roof which was approximately 18 foot from a lower level were exposed to fall  hazards while conduct roofing work without fall protection such as but not limited to using Personal Fall Arrest System, scaffolds, safety nets or guard rails being used.
