29 CFR 1910.29(a)(3)(vii): All work levels 10 feet or higher above the ground or floor shall have a guardrail of 2- by 4-inch nominal or the equivalent installed no less than 36 inches or more than 42 inches high, with a mid-rail, when required, of 1- by 4-inch nominal lumber or equivalent.  a. On or about March 11, 2013 - at the above addressed jobsite, employees conducting window washing activities from a manually propelled mobile scaffold, approximatley sixteen (16) feet above the ground, were not protected from falls by a guardrail system, thereby exposing the employees to fall hazards.
