29 CFR 1910.178(p)(1): Powered industrial truck(s) with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s):     (a)             Amcor Rigid Plastics USA, Inc. at 4990 Ironton Street, Denver, CO 80239: On and before 3/13/13, the employer did not ensure that powered industrial truck(s) with defect(s) or in any way unsafe had been withdrawn from service until restored to safe operating condition(s). Employee operated aYale model propane powered industrial truck while the left clasp to secure the tank was broken. The employer did not ensure that the powered industrial truck was taken out of service. 29 CFR 1910.303(g)(1)(i):  The dimension of the working space in the direction of access to live parts operating at 600 volts or less and likely to require examination, adjustment, servicing, or maintenance while alive was less than indicated in Table S1:   (a)      Amcor Rigid Plastics USA, Inc. at 4990 Ironton Street, Denver, CO 80239: On and before 3/13/13, the employer did not ensure the working space of at least 3 feet in front of electrical panels in the warehouse in that the panels were blocked by a railing, table and other equipment.  1.    Breaker in the injection molding area  2.    Breaker in the color matrix area  3.
