29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:    On or about March 13, 2013, an employee was exposed to a fall hazard of approximately 20 feet, while climbing in and out of a Star Industries Star "B" Series Safety Work Platform attachment to a fork lift (JCB Telehandler, Model 532 Turbo, Serial # 0777372) from the metal roof.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about March 13, 2013 employees were exposed to a fall hazard of approximately 20 feet while renovating a metal roof without fall protection.     THE JD CONSTRUCTION COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(B)(10), WHICH WAS CONATAINED IN OSHA INSPECTION NUMBER 314300245, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRIRMED AS FINAL ORDER ON JULY 19, 2010, WITH A FINAL ABATEMENT DATE OF JULY 7, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 10103 PERRIN BEITEL RD, SAN ANTONIO, TX 78207.
