29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  On or about 03/14/2013, the employer did not ensure that walkways and work spaces were kept clear of construction debris that was accumulating on the floors creating tripping and fall hazards. Construction debris and framing on/in hallway and individual rooms creating tripping hazards.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about 03/14/2013. the employer did not ensure that employees working in areas where there were suspended fixtures present, were provided with and were using head protection such as hard hats. lighting fixtures were hanging from the drop ceiling frame to an elevation of 70 inches with sharp edges exposed.
