29 CFR 1910.157(e)(3): The date of the annual maintenance check of portable fire extinguishers was not recorded:   This violation occurred on March 18, 2013 at a jobsite at Camp Smith, New Fitness Center Project where equipment operators were expected to use fire extinguisher in the event of a fire. The fire extinguishers in the heavy equipment such as the Caterpillar/Dozer DZ DDCT and Skid Steer 289C were not inspected and recorded.
