29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) Construction Site: On or about March 18, 2013, and at times prior thereto, as the controlling employer, Envision Commercial Construction Division of BTW Inc., did not ensure its sub-contractor, Castlerock Construction Inc., protected their employees in that employees were exposed to a fall of approximately eleven feet to the ground below while walking the top plate to install prefabricated trusses on a new residential home.    (b) Construction Site: On or about March 18, 2013, and at times prior thereto, as the controlling employer, Envision Commercial Construction Division of BTW Inc., did not ensure its sub-contractor, Castlerock Construction Inc., protected their employees in that employees were exposed to a fall of approximately nine feet to the ground below while working in and around unguarded floor openings.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    (a) Construction Site: On or about March 18, 2013, and at times prior thereto, as the controlling employer, Envision Commercial Construction Division of BTW Inc., did not ensure its sub-contractor, Castlerock Construction Inc., protected their employees in that employees were exposed to fall to the floor below using a stairway with approximately fourteen risers to access/egress the second story from/to the ground level to retrieve tools and material.
