29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    WORKSITE: The employee installing a sign on the buildings exterior wall from a fabricated frame scaffold was not protected by an appropriate fall protection system, exposing to a fall approximately 13 feet to the ground.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold and/or aerial lift trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold or aerial lift being used and to understand the procedures to control or minimize those hazards.     WORKSITE: The employee working from a fabricated frame scaffold and the basket of a truck mounted articulating boom aerial lift was not provided training in the recognition of hazards associated with the type of scaffold and/or aerial lift being used and the procedures necessary to eliminate or minimize those hazards.

29 CFR 1926.453(b)(2)(v): A safety harness shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.    WORKSITE: An employee was not wearing a safety harness while riding the basket of a truck mounted articulating boom aerial lift to access the platform of the fabricated frame scaffold to/from the ground.                                                    NW Sign Industries Inc. was previously cited for a violation of this occupational safety and health standard (29 CFR 1926.453 (b) (2) (v)), which was contained in OSHA inspection number 313104432, citation number 1, item number 1, affirmed as a final order on 9/11/2009, with respect to a workplace located at 200 S Pine Island Road Plantation, FL 33324.
