29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On March 18, 2013,  at the job site on the South corner of the apartment building, employees were working on the roof with a slope 4:12 without fall protection at a height that was greater than 6 feet, exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid surface.    At the job site, on March 18, 2013, employees were using a ladder to access a roof, which did not extend 3 feet above the landing surface and was not properly secured at the top, exposing employees to fall hazards at an approximate height of 18 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    On March 18, 2013, at the job site on the South corner of the apartment building, employees were working on the roof without any fall protection. The employees did not  receive any fall protection training and were exposed to fall hazards at an approximate height of  18 feet.
