29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a). Worksite located at I-35 & Flintlock Road, Liberty, Missouri- Employees were observed working and walking underneath elevated employees and were not protected from impact by the use of hard hats.

29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:     (a). Worksite located at I-35 & Flintlock Road, Liberty, Missouri- Employees were observed working from an aerial lift and were not utilizing fall protection equipment.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:     (a). Worksite located at I-35 & Flintlock Road, Liberty, Missouri- Pier #8- Employees were observed working and walking on top of a bridge pier without the use of any means of fall protection.     (b). Worksite located at I-35 & Flintlock Road, Liberty, Missouri-Pier #7- An employee was observed on the edge of the bridge decking and was not protected from falling by the use of any means of fall protection.
