Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to struck by hazards.      a. On or about March 14, 2013, an employee was injured by an employer manufactured tip adaptor on a Jennings Model 8108 Horizontal PTFE Extruder during a pre-compression phase operation.    Feasible and acceptable methods of abatement include, but are not limited to:    1. Implement a preventative Maintenance program to include tip adaptors for Jennigs horizontal PTFE extruders.    2. Use of Original Equipment Manufacturer provided tip adaptors for Jennigs horizontal PTFE extruders.
