29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about March 18, 2013, west side of the construction site, employees using an extension cord to energize a Makita skill saw that had the outer sheathing pulled away exposing the internal wiring were exposed to electrical shock hazards.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On or about March 18, 2013, on the west side of the construction site, an employee elevated on a wooden platform by the forks of a JLG Sky Track to perform decking activities was exposed to a fall hazard of approximately 20 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about March 18, 2013, on the west side of the construction site, an employee was exposed to a fall hazard of approximately 20 feet while engaged in decking activities on a commercial construction site from a work platform without fall protection.
