29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about March 19, 2013, at an oil site on Mitchell 8014 in Orla, TX, employees were living in crew living quarters where ground rods that provide grounding to the electrical services of the trailer were not long enough and were not buried in sufficient lenght. Employees were exposed to potential electrical hazards.
