29 CFR 1926.307(d)(1): Pulley(s) with part(s) seven feet (2.128 m) or less from the floor or work platform were not guarded in accordance with the requirements specified in paragraphs  (m) and  (o) of this section:Note:  Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than 6 ft. 6 in. (1.976 m) from the floor or platform may be guarded with a disk covering the spokes.   On or about March 18, 2013, the employer did not ensure the guards on the pulleys were in place on the following machines prior to employee's use.  Employees were exposed to flying chip hazards.    a)  The Rigid Air Compressor located on the west side of the building.   b)  The Concrete mixer on the north side of the building.

2CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.   On or about March 18, 2013, the employer did not ensure the tubular welded frame scaffold had guardrails on all open sides and ends to protect employees from falling.   Employees were exposed to fall hazards of approximately 19 feet to the ground below.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.   On or about March 18, 2013, the employer did not ensure the tubular welded frame scaffold was erected with toe boards to protect the employees working on lower levels and on the ground from falling debris and hand tools.  Employees were exposed to struck by hazards.
