This paragraph requires the employer to develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use.     Employees engaged in metal cutting activities in the yard using half mask cartridge respirators were exposed to respiratory hazards in that a comprehensive written respiratory program had not been developed and implemented to include training, respirator storage, respirator maintenance and pulmonary function and fit testing.

Employees engaged in parts cleaning activities using the solvent tank at the west wall of the maintenance shop were exposed to corrosive hazards in that an eye wash was not available at the facility.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.    Employees engaged in grinding activities at the pedestal mounted abrasive wheel in the maintenance shop were exposed to struck-by and caught-between hazards in that the grinding wheel was not equipped with a work rest.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)     Employees engaged in grinding activities at the pedestal mounted abrasive wheel in the maintenance shop were exposed to struck-by and caught-between hazards in that the tongue guard on the grinding wheel was not properly adjusted to within 1/4 inch.

Such equipment may consist of pails of water, buckets of sand, hose or portable extinguishers depending upon the nature and quantity of the combustible material exposed. 29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions.    Employees engaged in welding activities in the yard were exposed to electrical shock hazards in that the insulation surrounding the cable approximately 44 inches from the hot lead was damaged and repaired using black tape.
