(a) Westar Framing, 3100 Pearl Street, Boulder, CO 80108: On and before March 14, 2013 the exposing employer did not ensure that toeboards, when used as falling object protection, were erected along the edge of the overhead walking/working surface of ladder openings in buildings C and D for a distance sufficient to protect employees below. This condition exposed employees to struck-by hazards.
